House calls are made at our doctor’s discretion to regular patients of the practice only.
Nursing home
Our doctors see patients at Sarah Claydon Nursing Home in Milton. Please contact the practice if you wish for one of our doctors to take on a patients’ care.
Personal Information
We have a strict policy for handling patient information which accords with national privacy legislation. The information that you provide is strictly confidential and accessible only by staff (who are subject to a confidentiality agreement as a condition of their employment). Your personal information will not be released to third parties without your express written consent (or your parent/guardian's consent for a child under 16). If you require a copy of our privacy policy, please ask at Reception.
Communication Policy
If you contact the practice and want to speak to one of our doctors, our receptionist will take your details so that the doctor can ring you back. We will only interrupt the doctor’s consultation if the problem is urgent. As most issues are best dealt with in a consultation we encourage you to schedule an appointment at a convenient time.
Test Results
As a rule, we will not provide results over the phone. All patients are encouraged, at the time of the investigation to make further appointments to discuss results. However, we do have a recall system and you may be contacted at a later date to make an appointment with your doctor to discuss results.

We believe that patients share responsibility for ensuring they carry out tests that were requested and to also follow up on test results.
Recalls
Patients have access to the diabetic register, pap screening register, bowel screening register, health assessments and management of chronic diseases.

Once registered, patients are routinely recalled. You may advise your doctor if you object to being included in reminder systems or registers.
Repeat Prescriptions
You will need to book an appointment with your doctor for a repeat prescription for the proper management of your medication.

Appointment Cancellation Policy - Milton Family Medical Practice
Please provide a minimum of 2 hours' notice if you cannot attend your appointment. This provides us with the opportunity to offer your appointment to another patient who may need to see a doctor urgently.

The practice reserves the right to charge a ‘No Show’ fee of $35.00 if you fail to cancel your appointment within the requested time. Repeated no shows without notice may result in a restriction being placed on your patient file, and you will no longer be able to make appointments with the practice.
No Show Policy - South Coast Skin Cancer Clinic
If you fail to attend your scheduled skin procedure at the South Coast Skin Cancer Clinic with no notice of cancellation, you will be deemed as a ‘No Show’ and charged a $50 ‘No Show’ fee.
A receptionist will contact you to re-book your procedure. On the re-scheduled day of your procedure, Reception will take pre-payment upon your arrival of both the ‘No Show’ fee and your quoted procedure cost.
